Action Plan - How to Generate the Report for Health Clinics
The Action Plan is a personalized report that is generated during a health clinic for the patient to take home. It includes their test results vs their targets and personal recommendations.
(NOTE: The defaults setup must be complete prior to booking appointments. See 'Edit Action Plan Recommendation Defaults')

How It Works:
After you have gone over the Health Profile Questionnaire & Online Booking Questionnaire and if applicable complete the Appointment Assessment questionnaire with the patient, the next steps are (1) to complete the Health Clinic Test Results vs Target Section, (2) personalize the recommendations and then (3) Generate the Action Plan. The Action Plan is usually generated at the end of the appointment.

(1) Health Clinic Test Results vs Target:
Below the questionnaires on the details tab of a health clinic appointment you will find the link to open up the Test Results vs Targets. This section contains the the tests that are being conducted during this appointment (these were setup in advance).
- There is where we record the patients test results vs their target. When you click into one of the fields, a tool tip will present itself on the right so you have an easy reference point when determining what to recommend as their personal target.
- When you are done, hit save. You will see the 'Saved' time stamp on the details tab of the appointment update with the time these values were saved.
- If you need to edit, you can open the window back up and re-save at anytime.

(2) Recommendations:
On the Action Plan tab of a health clinic appointment you will find all of the default recommendations for this service line based on the different possible test results (these were setup in advance). Now its time to personalize the recommendations to suit your patient.
- To select a point to be included in the action plan for your patient, simply check the box beside it.
          ~ You can edit the point further by clicking the edit button the right side of the line. Edit the recommendation and click save. This will now be saved to this patients appointment only.
          ~ If you don't select any points from a certain section, that section will not be included in your patients action plan.
- You can also edit the section Names and Footer if needed.

(3) Check Doctor Contact Info
There is a 'doctor' section near the top of the tab.
- If the contact information is not present, use the Doctor Lookup link to find it.
- When you find it, use the Edit link to input the information into the system. This will be saved to the patient profile for next time.
Of Note:
You may wish to send the Action Plan to a patients family doctor if you wish to submit it as a Pharmacist's Recommendation [Pharmaceutical Opinion Program (POP)] that could for example result in a change to the patients prescription.

(4) Generate the Action Plan

Once you are satisfied with the patients Test Results vs Targets and their Recommendations, you can generate a report for the patient.
- On the Action Plan tab of a health clinic appointment, at the top there is a box that contains reports relating to this health clinic.
- Make sure Action Plan is checked on > then hit generate.

- Then you want to hit either Print, Email or Fax.
..... If you hit Print:
It merges all the reports into one PDF and opens it up so they can be sent to the printer all at once.
..... If you hit Email: It pops open a window where it displays the patients email for verification (or you can change it if you wish to send to someone else) > then hit 'send'. The recipient will receive an email with the report(s) attached.
..... If you hit Fax: It pops open a window where it displays the default fax cover page copy. You can edit the fax cover page copy as you wish and then hit print. It will then generate a complete fax cover page which includes your header, today's date, Attn: patients doctor name & fax number and your footer. It merges the cover page and the report into one PDF that can now be printed and faxed to your patient's doctor.
