Questionnaires
There are 4 types of questionnaires...

Online Booking Questionnaire: 
This questionnaire is meant to be filled out by the patient while they are booking their appointment online. The thought is to keep these questions service line specific and relatively simple with the aim to not scare the patient away with too many questions or too much detail.
Health Profile Questionnaire:
Once the patient has booked their appointment they will receive a confirmation email with a request to fill out their health profile questionnaire. 
Appointment Assessment Questionnaire:
This questionnaire is meant to be filled out by the health care professional during the appointment. Certain questions are often required to be asked and recorded at the time of the appointment, they can be managed here.
Follow-up Questionnaire:
This questionnaire can be used for follow-up appointments and replaces the online booking questionnaire. This allows you to save the original online booking questionnaire for reference, and ask the patient new questions for follow-ups.
This must be enabled per service line, which is explained here.

NEW! Monitoring Questionnaire:
This questionnaire can be used for monitoring the patient after their appointment is completed. This will be sent to the patient in a schedule of emails and saved to the patient's appointment on the monitoring tab. This must be enabled per service line, which is explained here.

- All question's are completely customizable and have a number of options that go along with them which are explained below.
- Any changes made to a question will be applied to new appointments only. To update legacy (previously answered) questions use the check box under the save. Limited options are available for legacy edits.
- All patient questionnaires are accessible from the patients appointment screen. Click the Edit button to edit the questionnaire for a patient. 

All questionnaires are managed separately:
Go to the Setup menu > Questionnaires > select the questionnaire. 


List View:
Once you have selected the questionnaire from the setup menu you will be presented with a list of questions that belong to that questionnaire. 
The columns explained from left to right:
Question Type: Template or Custom: Template questions are managed by your service rep. These are questions that are required to be answered to complete and generate certain reports.
Answer Type: We offer 6 different answer types; Text Box
, Checkbox List, Multiple Choice, Column Input, List Input & Calculation. They are explained below.
Service Lines: Questions are service line specific, and can be shared between multiple service lines. All service lines that share a certain question will be listed in the Service Line column. 
Additional features on the page:

- Print / Filter: You can filter this list by selecting a service line from the drop menu. This enables the print button so you can print your questionnaire if needed for walk ins or patients without internet access. Filtering also makes it easy to get an overview of your questions for a certain service line.
- Re-ordering: You can re-order the questions by using the up and down arrows on the far right side.

Editing a Question:
To edit a question you simply click the 'edit' button beside the question on the right hand side. Question options are explained below. 
(Note: You will not be able to edit template questions. Please contact your service rep if needed.) 

All Questions:
- Active Check box: The active check box is used to turn a question on or off. If the check box is unchecked, the question will be considered inactive. This means it will no longer appear in the questionnaire to the patients.
- Show on appointment screen: Should generally always be checked on. This ensures you will see a recap of the question on the appointment screen if the question has been answered.
- Mandatory Check box: If the mandatory check box is checked the question will be marked as mandatory and required to be answered by the patient. 
- Short Question: This is used to re-cap the full question and displayed on the appointments detail tab. For example, if the full question is 'Please list all of your allergies below:' the short question could be 'Allergies:'
- Service Line Selection: Check on all service lines that should ask this question, and this question will be included for that service line.

Answer Type:

- Text Box: Means that the patient can enter anything their answer into a text box.
There is the option to specify whether the answer should be text, integer number or decimal number. Show Alert: Check this on if you wish to have an alert displayed in the appointment details recap if the patient selects this option. You can specify whether it should be flagged based on any answer or an answer with 5+ words. You can add an internal note to also be displayed if needed.

- Checkbox List:
 Means that the patient can select one or more predefined answer. 

Multiple Choice: Means that the patient can only select one predefined answer. 
You can add Answer Options by using the 'Add Answer Options' button. To edit preexisting answer options, use the check box to select it and click edit (or delete). To 'arrange the list alphabetically' click that button. To move the options one by one use the arrows on the right. Each answer option has a number of configuration options:
Show Alert: Check this on if you wish to have an alert displayed in the appointment details recap if the patient selects this option. You can add an internal note to also be displayed if needed.
Explanation: Check this on if you would like the patient to be required to enter an explanation if they select this option.

The Online Booking Questionnaire has these additional configuration options:
Prevent Booking: Check this on if you would like to prevent the booking from happening if the patient selects this option. You can add a note to be displayed as an alert to the patient if they select this option. 
Adjust Appt Length:
COMING SOON! Check this on if you would like to adjust the appt length if the patient selects this option. You can specify + or - and by how many minutes.

- Column Input:
Means that the patient is supposed to enter their answer into predefined columns that go along with your question. (IE: Please specify your current medications, dose and side effects:)
You can define the number of columns and rows, and give each column header a name. There is the option to specify whether the answer should be text, integer number or decimal number. Show Alert: Check this on if you wish to have an alert displayed in the appointment details recap if the patient answers this question. You can specify whether it should be flagged based on any answer or an answer with 5+ words. You can add an internal note to also be displayed if needed.

- List Input:
Means that the patient is supposed to enter their answer next to items in a predefined list. (IE: Please specify what you usually eat for the following meals:)
You can add Answer Options by using the 'Add Answer Options' button. To edit preexisting answer options, use the check box to select it and click edit (or delete). To 'arrange the list alphabetically' click that button. To move the options one by one use the arrows on the right. There is the option to specify whether the answer should be text, integer number or decimal number. Show Alert: Check this on if you wish to have an alert displayed in the appointment details recap if the patient answers this question. You can specify whether it should be flagged based on any answer or an answer with 5+ words. You can add an internal note to also be displayed if needed.

- Calculation:
Means that the patient is supposed to enter a number as their answer next to items in a predefined list. (IE: Please rate the following on a scale of 1-10:)
You can add Answer Options by using the 'Add Answer Options' button. To edit preexisting answer options, use the check box to select it and click edit (or delete). To 'arrange the list alphabetically' click that button. To move the options one by one use the arrows on the right. There is the option to specify whether the answer should be integer number or decimal number. Show Alert: Check this on if you wish to have an alert displayed in the appointment details recap if the patient answers this question. You can specify whether it should be flagged based on any answer or if an answer is above a certain total. You can add an internal note to also be displayed if needed.

- NEW! Rating: Means that the patient is supposed to select a number rating. You can configure the number of options, and low/ high label (IE: Please rate the following on a scale of 1-10, where 1 is Poor and 10 is Excellent.) You can calculate the total if needed, and for the Monitoring questionnaire only you can save the previous answers which will display the patients progress. Show Alert: check this on to trigger an alert based on the patient's answers and either the total if being calculated or individual ratings. Add list options by using the 'Add List Option' button above.
