Consent Form
Consent Form:
Before dispensing a drug or providing a service a consent form is often required.
How it works:
- The consent form can be generated from any appointment, from either the prescription tab, or the patient reports button.
- Make sure the check box is checked beside 'Consent Form' and hit the generate button.
- Once it is generated it will be saved into the system and accessible anytime.
- You can then print and have the patient sign.
Included in the report:
- Patient ID: Name, DOB & PHN
- Will be dispensing: Dynamic sentence that recaps the appointment date & time and the health care professional's name and title of who is logged into the system at the time the report is generated.
- Drugs. All drugs will flow in from the prescription tab into 1 of 2 charts. There is one for open and dispensed drugs, and one for deferred and rejected drugs.
~ Open and dispensed drugs will appear under the 'will be dispensing line', and the chart will recap the drug name, dose size and authorized by.
~ Authorized by is determined from the prescribing authority configuration in the user profile, and/or the medical directive status of the drug at the time the prescription is generated.
~ Deferred and rejected drugs will appear in a separate chart so you can have the patient sign off on that as well.

- Consent Form Copy: This is pulled from the service line's consent form defaults.
- Consent Options: These are pulled from the service line's consent form defaults. They will be presented to the patient online when requesting their digital signature. If present, the patient will not be able to proceed without selecting an option. They will also be printed on both the consent form and the digital consent & summary report.
- Signature Field: This is where the patient is meant to sign.
- Emergency Contact: If this information is present in the patient profile is will get pulled from there.
- Footer: This gets pulled from the service line's consent form defaults.
If any sections are not present, that section will just not be included in the report.
To edit the consent form defaults:
Go to the Setup menu > Service line setup > Select the service line > go to the 'Reports' tab > Click 'Edit Defaults' from beside 'Consent Form'.
PLEASE NOTE:
- The consent form will not contain the digital signature. To print the consent form with digital signature, the Digital Consent & Summary Report must be used instead. This is because the drugs are included on the consent form with the specified deferred or rejected drug status, and optionally could include the drug instruction notes. We cannot present this information to the patient before their appointment, and therefore the digital signature cannot be applied to this consent form.
