Complete Questionnaire Report

Complete Questionnaire Report
This is a report that can be generated when needed. It is meant for patients who do not have access or otherwise are unable to fill the questionnaire out online.

How it works:
Go to the Reports Menu > Complete Questionnaire.
> Select the service line you need the questionnaire for.
> Hit the 'Generate Report' button.
> PDF will open, you can print and give to the patient to fill out. (You can enter the answers online later, or scan the document and upload to the patient's upload tab.)

Included in the report, in this order:
- Patient profile
- Trip Itinerary (if applicable)
- Online Booking Questionnaire
- Health Profile Questionnaire
- Appointment Assessment Questionnaire (Only questions that have 'Display to patient on walk-in site' selected will be included in the report)
